Incidents

Customize your incident form

The Incident Form Editor sets which fields your team must fill in, which are hidden, what your legal disclosure says, and what it takes to close a claim.

October 15, 2026

A tunnel with cameras on every bay needs different evidence from a self-serve site. The form editor is where you make the claim form match how your wash actually handles damage.

Before you start

  • Available on the Bundle plan, or on a Custom plan that includes Incidents.
  • Only Admins and Super Admins open the editor. Team Members never see it.
  • The settings are company-wide, so they apply at every one of your locations.
  • Changes apply to new claims. A claim already logged keeps the form it was created with.

Open the editor

Select Incidents in the side menu, then Form Editor. The page is six sections you can collapse, each listing its fields with Field, Field type and Settings.

Set what each field does

Every configurable field has two switches:

  • Required makes your team fill it in before the claim saves.
  • Hide takes the field off the form entirely.

The two cannot both be on. Turning on Hide turns Required off.

Some fields carry a badge instead of switches. Always Required fields can never be changed or hidden: Customer Name, Email, Phone, Year, Make, Model, Incident Date and Incident Time. A conditional field marked Required When Shown has to be filled in whenever its parent question is answered Yes.

Write your legal disclosure

Under Legal Disclosure, type the text your customers acknowledge before they submit a claim. It runs to 5,000 characters, with a counter under the box, and leaving it blank falls back to WashUp's wording.

The same section holds Require customer signature, and under it the option to hide the tracking link that can go in the customer's confirmation email.

Decide what it takes to close a claim

Resolved & Denied Requirements is the list your Admins must complete before a claim can be marked Resolved or Denied. Each row has one switch, Required / Show, and you set the two outcomes separately. The rows are the note, the reason, the evidence relied on, and how the customer was told.

These appear on the claim when someone closes it, not during intake.

Check it, then save

Select Preview Incident Form to open the form with your unsaved settings. It is fully interactive so you can work through it end to end, and nothing you type in there is saved. Return to Form Editor brings you back with your settings intact.

Select Save Form Configuration when you are happy. Nothing is stored until you do.

Good to know
  • Resolution Details is not part of intake. It appears on a claim after it is created and can be filled in any time before the claim closes.
  • Linked Incidents is always shown and cannot be switched off. It is its own section on the form.
  • Hiding a field does not delete what old claims recorded in it. The field just stops being asked for.
